This policy explains what information Artistry Clinic collects, why we collect it, how long we keep it, and what you can ask us to do with it.

Two kinds of information
Ontario law treats health information differently from ordinary personal information, so this policy separates them.
Personal health information
Anything you tell us about your health, medical history, medications, allergies, treatments received, and clinical photographs. This is governed by Ontario's Personal Health Information Protection Act, 2004 (PHIPA). We collect it only to assess whether a treatment is appropriate for you, to plan and deliver that treatment, and to keep the clinical record the law requires us to keep.
Personal information
Your name, phone number, email address, appointment history, and payment records. This is governed by the federal Personal Information Protection and Electronic Documents Act (PIPEDA). We collect it to book appointments, take payment, and contact you about your care.
What we collect through this website
This site is a static website. It does not use cookies for tracking, and it does not run analytics or advertising scripts on the page.
If you send us a message through a form on this site, we receive what you type plus your email address. If your message contains health information, we handle it under PHIPA.
Who we share it with
We do not sell your information. We share it only in these situations:
- With other healthcare providers involved in your care, when you consent
- With payment processors, limited to what is needed to complete a transaction
- Where a law, court order, or regulatory college requires disclosure
- Where there is a risk of serious harm, as PHIPA permits
How long we keep it
Your rights
You can ask us to:
- Give you a copy of your record
- Correct information you believe is inaccurate
- Explain how a specific piece of information was used or disclosed
- Withdraw consent for a use that is not required for your care

If you are not satisfied with our response, you can complain to the Information and Privacy Commissioner of Ontario for health information, or to the Office of the Privacy Commissioner of Canada for other personal information.

Changes to this policy
We will post any change on this page and update the effective date above. Material changes affecting how we use health information will be brought to your attention before they take effect.
